Release of Liability – Children

I/we as parents or legal guardians relinquish all liability against St. Demetrios Greek Orthodox church (including pastor/youth advisors, chaperones) or anyone affiliated with St. Demetrios Greek Orthodox church with regard to the following:

• any activities sponsored by any of the organizations of the Parish of St. Demetrios
• any sports activities sponsored by any of the organizations of the Parish of St. Demetrios
• any injury sustained from either of the above, or during transportation to and from any field trips or excursions sponsored by the Parish of St. Demetrios

I/we agree to hold harmless St. Demetrios Greek Orthodox Church and any organizations of the Church in the event of any accident.
